29 CFR 1910.178(g)(2): Battery charging apparatus for industrial trucks located in battery changing and charging installation was not protected from damage by trucks:    a)  In the warehouse - Employees were operating two industrial fork trucks where three battery chargers ("Ferro Five" S/N: JPIU937335, 24 volts/50 amperes; "Exide Gold" S/N: DA112988, 36 volts/152 amperes; and "Lifeguard" S/N: 99U15693, 36 volts/135 amperes) were not protected from damage by the industrial fork trucks, creating contact with electricity hazards.       NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:    a) In the warehouse - Employees were using a flexible cord as permanent wiring to power appliances, such as but not limited to, microwave and coffee maker, from outlet (~125 volts/15 amperes) creating contact with electricity hazards.    Everest Caster & Wheels, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.305(g)(1)(iv)(A), which was contained in OSHA inspection number 944909, citation number 1, item number 3 and was affirmed as a final order on 12/03/2013, with respect to a workplace located at 17815 Foster Rd #146 in Liberty, MO 64068.
