Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact. a. On or about August 1, 2014 - at the above addressed jobsite, employees using pneumatic nail guns to perform roofing/shingling operations did not wear safety glasses, thereby exposing employees to the eye injuries. CALDERONE ENTERPRISES, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.95(a) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 945255, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON DECEMBER 31, 2013 WITH RESPECT TO A WORKPLACE LOCATED AT 2191 ALTA VISTA, NEW LENOX, IL 60451.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. a. On or about August 1, 2014 - at the above addressed jobsite, employees performing roofing operations approximately eight (8) feet ten (10) inches above the ground were not protected from falls, thereby exposing the employees to fall hazards. b On or about August 1, 2014 - at the above addressed jobsite, employees conducting roofing operations were exposed to falls to the inside of the building created by the presence of unprotected skylights.
