29 CFR 1910.212(a)(3)(ii): The  point of operation of machines whose operation exposed an employee to injury, was not guarded:  a.  On or about August 5, 2014, at Cell #6 Sub Assembly area, the MSC milling and drilling machine with serial number 83001938 did not have a guard at the point of operation while an employee was drilling metal parts, exposing the employee to caught-in and struck-by hazards.  b. On or about August 5, 2014, at Cell #6 Sub Assembly area, the Rockwell drilling machine with serial number 124-8119 did not have a guard at the point of operation while an employee was drilling metal parts, exposing the employee to caught-in and struck-by hazards.  c. On or about August 7, 2014, at Cell #7 Sub Assembly area, the Famco drilling machine with serial number D-B 1P00 did not have a guard at the point of operation while an employee was drilling metal parts, exposing the employee to caught-in and struck-by hazards.

29 CFR 1910.107(b)(5)(i): The spraying operations was not designed, installed or maintained so that the average air velocity over the open face of the booth not be less than 100 linear feet per minute:  a. On or about August 5, 2014, at paint area #1, the employer did not endure that the average air velocity over the open face of the booth was not less than 100 linear feet per minute while employees sprayed flammable coating to aircraft parts.  The average air velocity measured at the open face of the booth was 75.4 linear feet per minute.

29 CFR 1910.134(d)(1)(ii): The employer did not select and use a NIOSH-certified respirator in compliance with the conditions of its certification:  a.  On or about August 5, 2014, at paint area #1, the employer did not ensure that painters used a NIOSH-certified respirator while spraying a flammable coating inside the spray booth.  The breathing air hose provide by the employer was not part of the NIOSH certification for the 3M air supplied system used by the painters.
