29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: On 8/6/14, an employee working in an excavation did not have a safe means of egress because the sloped end of the trench was obstructed by crossing utilities.

29 CFR 1926.1053(b)(16): Portable ladder(s) with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately removed from service. On 8/6/14, a ladder used for access/egress to the trench/excavation had been cut off at both the top and bottom and had a split rail on one side at the top of it.
