Instance 2: The employer/owner who used a pneumatic nail gun for framework and his employee were not wearing eye protection.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  Location: 829 Boston Post Rd, Milford, CT 06460  The 39'8" wide by 52'3" long, low sloped/flat roof with a 10' fall distance to ground was not equipped with a fall protection system.
