29 CFR 1910.304(b)(3)(i): All 125-volt, single-phase, 15- and 20-ampere receptacles or equipment used was not installed with ground-fault circuit-interrupter protection for personnel. On or about July 31, 2014, employer did not ensure that a 65 gallon oil dispenser was equipped with a ground fault circuit-interrupter as per manufacturer's requirements. This condition exposed employees to electric hazards.
