29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: (a). At a commercial construction site located at 5430 45th Street in Bradenton, employees elevated in the basket of an aerial lift installing the roofs facia were not attached to the boom or basket of the aerial lift exposing them to a fall hazard of 18 feet on or about August 6, 2014.
