Greene Point Health & Rehabilitation, Union Point, GA - In the kitchen, employees washing and rinsing cooking utensils were exposed to slippery conditions. A slip resistant mat was not provided at all times when employees were working in front of the triple sinks, on or about 8/5/14.

29 CFR 1904.29(b)(3): Each recordable injury or illnesses was not entered on the OSHA 300 log and/or incident report (OSHA Form 300A or equivalent) within seven (7) calendar days of receiving information that a recordable injury or illness has occurred: Greene Point Health & Rehabilitation, Union Point, GA - On 7/14/14, an employee experienced a recordable injury after slipping on the floors. The employee had days away from work and restricted activity due to the injury. The injury was not recorded in the 2014 OSHA 300 log, on or about 8/5/14.

29 CFR 1910.1030(f)(1)(ii)(D): The employer did not ensure that all medical evaluations or procedures including the hepatitis B vaccine, vaccination series, post-exposure evaluation, or follow-up, including prophylaxis, were provided according to recommendations of the US. Public Health Service current at the time these evaluations and procedure took place: Greene Point Health & Rehabilitation, Union Point, GA - Employees, such as, CNA's, LPN's, RN's, Housekeepers, and Laundry staff, were potentially exposed to blood and body fluid. The titer test for antibody Hepatitis B surface antigen, one to two months after the completion of the three-dose vaccination series, was not offered to the employees, on or about 8/5/14.
