29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition.  a)    At the recycling facility in Unadilla, GA: On or about August 5, 2014, and at times prior thereto, the employer exposed employees to fire hazards, and trips and falls by allowing the accumulation of solid-waste plastics, metal, cardboard and paper on floors and passageways, and around machinery throughout the facility.

29 CFR 1910.23(d)(1)(iv): Stairway(s) more than 44 inches wide, but less than 88 inches wide having both sides open, did not have one stair railing on each side:   a)    At the recycling facility in Unadilla, GA: On or about August 5, 2014, and at times prior thereto, the employer exposed employees to trip hazards and falls of up to 34 in. to the ground, in that a 5-riser stairway on the west-dock was not provided with adequate standard railings as employees accessed and egressed the dock platform.

29 CFR 1910.37(b)(2): Each exit was not clearly visible and marked by a sign reading "Exit":   a)    At the recycling facility in Unadilla, GA: On or about August 5, 2014, and at times prior thereto, the employer exposed employees to burns and smoke-inhalation hazards, in that an EXIT sign was not placed above the pedestrian door in the northwest corner of the building in order to evacuate the building during emergencies not limited to fire.
