29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a. 663 Exteriors, LLC on August 7, 2014, at the address of 211/213 Lisa Ann in Huron, Ohio: Employees were exposed to falls of approximately nine feet in that an extension ladder used to access the roof of a residential structure was used without extending the ladder's side rails at least three feet above the landing surface.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a. 663 Exteriors, LLC on August 7, 2014, at the address of 211/213 Lisa Ann in Huron, OH: The employer did not ensure employees were protected from falls of approximately nine feet, and up to approximately 15 feet while performing weatherproofing activities on a residential structure. 663 Exteriors, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 901914, citation number 1 item number 1 and was affirmed as a final order on 5/30/2013, with respect to a workplace located at 16909 Clifton Blvd., Lakewood, OH 44107.
