29 CFR 1926.453(b)(2)(v): A body belt was not worn with a lanyard attached to the boom or basket while working from an aerial lift: A. Schwartz Construction LLC worksite located at 1624 Albon Rd., Holland, Ohio: On or about August 07, 2014, the employer did not ensure an employees personal fall arrest system was secured (clipped-off) to a JLG 400S Aerial Lift. The exposed employee was performing steel erection activities from the aforementioned aerial lift.
