RF Masonry LLC worksite located at 1624 Albon Rd., Holland, Ohio: On or about August 7, 2014, the employer did not ensure the scaffolding level which employees were working was fully planked. Multiple areas of the scaffold were not fully planked exposing employees to a 20.00 foot fall while performing masonry activities.

29 CFR 1926.451(e)(1): Scaffold platforms more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used. A. RF Masonry LLC worksite located at 1624 Albon Rd., Holland, Ohio: On or about August 7, 2014, the employer did not ensure employees were provided with adequate access to the working levels of the scaffold system. Employees were climbing the tubular welded frames to access the scaffold approximately 20.00 feet above the lowest level.
