29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, pinch points, rotating parts, flying chips and sparks: a) On or about 8/1/14 at the Keeler Construction Co. maintenance shop located at 13519 W. Lee Road, Albion, New York; an employee was repairing an air brake canister on a company dump truck. The employee's hand was on the canister when the air was released causing the caging bolt to retract into the canister. The tip of the employee's thumb was amputated when it was caught between a pinch point created by the canister and washer on the caging bolt. NO ABATEMENT CERTIFICATION REQUIRED
