Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees exposed to fires and explosions from uncontrolled well releases:     (a) On or about July 23, 2014, for the employee working in the derrick man's basket at the height of approximately 75 feet where the Geronimo Cart's cable was anchored at approximately 62 feet from the rig at the well site identified as the Siana Oil and Gas Tank 1-3 vicinity of Watford City, ND.     (b) On or about July 23, 2014, for the employee working in the derrick man's basket where the Geronimo Cart's cable was secured at the lower end utilizing only one saddle clamp at the well site identified as the Siana Oil and Gas Tank 1-3 vicinity of Watford City, ND.     Abatement Note: Among other methods, one feasible and acceptable method to correct this hazard would be to ensure that the Geronimo Cart and line is installed in accordance with the Geronimo's operators manual.
