29 CFR 1926.100(a): 1926.100(a): At this job site, employees exposed to possible head injury from falling or flying objects or from impact with hand tools, equipment and building materials during construction operations were not wearing hard hats.   The employer did not ensure that the employee/s working on the scaffold and on the ground wore hard hats. Employees were exposed to the hazard of being struck-by tools, materials and equipment falling from above.

29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide:  a. The walkway on the wooden scaffold on the north side of the building consisted of a single 2 x 6 (5.5 inches wide). Employees were exposed to a fall hazard of approximately ten (10) feet to the ground below.   b. The walkway on the wooden scaffold on the west side of the building consisted of a single 2 x 6 (5.5 inches wide). Employees were exposed to a fall hazard of approximately eight (8) feet to the ground below.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  No fall protection was provided on the working platform (scaffold) on the north side of the building. Employees were exposed to a fall hazard of approximately eleven (11) feet to the ground below.

The employer did not ensure that employee/s installing sheeting on the roof were protected from falls. The employee/s were exposed to a fall hazard of approximately thirteen (13) feet from the eave to the ground.

The employee/s accessing the roof were exposed to a fall hazard of approximately thirteen (13) feet from the eave to the ground.
