29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system. a) Garage roof of residence located at 4503 Sterling Pointe Drive, Acworth, Georgia. The employees were working from unprotected steep slope, 10 in 12 pitched, roof without fall protection. The employees were not protected from fall hazards of approximately 12-feet 1-inch as no guardrails or safety nets had been installed and the employees were not wearing personal fall arrest systems.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at it's top to a rigid support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the ladder: a) Garage of residence located at 4503 Sterling Pointe Drive, Acworth, Georgia. The employees, working from unprotected steep, 10 in 12, pitched roof, were exposed to fall hazards of approximately 19-feet 8-inches while mounting and dismounting an aluminum extension ladder. The side rails of the ladder, which had been set up at the peak of the garage roof to access the roof, did not extend at least 3-feet above the upper landing surface.
