29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On or about 8/7/14, employees installed sheathing onto an 8:12 pitched roof without any form of fall protection while exposed to fall hazards of 14 feet or greater.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  On or about 8/7/14, employees utilized extension ladders to gain access to a catwalk in the lower portion of the trusses without having the ladder extended 3 feet above the landing.
