29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:    a) On or about 08/05/14, on the left side of house roof at 613 Fargo Ave. , Buffalo, NY:  Employees accessed the roof by climbing up & down the frame of an electrically powered device used to transport bundles of roofing shingles from the ground up to the roof. The device looked similar to a ladder, but it was not designed for employees to climb. At times, employees climbed rungs on both sides of this ladder-like device to access/exit the roof.  No stairway, ladder, ramp, runway, sloped embankment, or personnel hoist was provided.   NO ABATEMENT CERTIFICATION REQUIRED

TURK MASTERS CONTRACTING LLC  WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS  OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, WHICH WAS CONTAINED IN OSHA INSPECTION  NUMBER 577339, CITATION NO. 1, ITEM NO. 3, AND WAS AFFIRMED AS  A FINAL  ORDER ISSUED ON 09/25/2012, WITH RESPECT TO A WORKPLACE LOCATED AT 143 LAKE STREET, HAMBURG, NY  14075.  29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) On or about 08/05/14, on the left side of roof of the house at 613 Fargo Ave. , Buffalo, NY:  Three employees engaged installing roofing shingles were exposed to a fall of approx.  18 foot. The roof pitch was approx.  12 in 12.  There were two rows of slide guards, one above the other, installed along the entire length of the roof eave, but  no  other  fall protection system was utilized.    NO ABATEMENT CERTIFICATION REQUIRED
