29 CFR 1910.134(c): The employer did not develop and implement a written respiratory protection program with required worksite-specific procedures and elements for required respirator use:    Onboard USCG Cutter POLAR STAR, Fuel tank 4-21-1-F:  The employer had not developed and implemented a written respiratory protection program.  This was evidenced when laborers were working inside the tanks wearing 3M 6899B full face respirators which hadn't been properly cleaned, the respirators had non-skid inside the face piece which could make the seal ineffective.

Onboard USCG CUTTER POLAR STAR, fuel tanks 4-21-1-F, 4-21-2-F:  Two laborers were cleaning inside diesel fuel tanks and the employer had not established a shipyard rescue team or arranged for an outside rescue team to respond promptly to a request for rescue service.

29 CFR 1910.134(h)(1): Respirators were not cleaned and disinfected using the procedures in Appendix B-2 of 29 CFR 1910.134 or equivalent procedures recommended by the respirator manufacturer:   Onboard USCG Cutter POLAR STAR, Fuel tank 4-21-1-F:  Two laborers were working inside the tanks wearing 3M 6899B full face respirators which had not been properly cleaned. The respirators had non-skid inside the face piece around the sealing surface.   All Star Cleaning and Preservation was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1910.134(h)(1)(i) which was contained in OSHA inspection number 869763, citation number 2, item number 1 and was affirmed as a final order on March 14, 2013, with respect to a workplace located at U.S. Coast Guard Base, Alameda, CA.

29 CFR 1915.152(a): The employer did not provide or ensure each affected employee with the appropriate personal protective equipment (PPE) for the eyes, face, head, extremities, torso, and respiratory system, including protective clothing, protective barriers, personal fall protection equipment, and life saving equipment, meeting the applicable provisions of Subpart I, wherever employees were exposed to work activity hazards that required the use of PPE.        Onboard USCG Cutter POLAR STAR, Fuel tanks 4-21-1-F, 4-21-2-F:  Three laborers were working inside the tanks washing down, potentially exposing the employees of getting diesel fuel on their skin. They were not provided Tyvek coveralls as required on the marine chemist certificate.
