29 CFR 1926.405(g)(1)(iii)(B): Flexible cords and cables were run through holes in walls, ceilings, or floors: a) At 702 North 8th Street, Reading, PA - On or about August 7, 2014, employees were exposed to fire and electrical shock hazards when heat welding plastic tubing using a McElroy Socket Heater powered with a flexible cord run through a hole in the floor because there were no power sources on the floor where they were working. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: a) At 702 North 8th Street, Reading, PA - On or about August 7, 2014, employees were exposed to fire and electrical shock hazards when heat welding plastic tubing with the McElroy Socket Heater powered with a flexible extension cord that had the outer insulation pulled at the outlet end so that there was no longer outer insulation or strain relief provided to prevent tension from being directly transmitted to joints or terminal screws. No abatement certification or documentation required.
