29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: Drill site: On or about August 6, 2014, and at times prior thereto, employees were working on Latshaw Rig #11. Behind the crew trailers were two 50-foot 12 AWG extension cords connected together and feeding power to three pieces of equipment. The label states that this is prohibited. This condition exposed employees to electrical shock hazards.

29 CFR 1910.305(a)(2)(xi): Cable assemblies and flexible cords shall be supported in place with supports in the form of staples, cables ties, straps, or similar type fittings installed so as not to cause damage. Drill site: On or about August 6, 2014, and at times prior thereto, the flexible cords installed under the trailers were not supported in place with the proper supports so as not to cause damage.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: Drill Site: On or about August 6, 2014, the strain relief to the sump pump of the water and sewer system, located behind the crew living quarters was damaged. This condition exposed the employees to electrical hazards.
