Signs of conspicuous and easily read style giving instructions for the use of the manlift were not posted at each landing or stenciled on the belt: a) At the Humphrey Manlift Elevator, On or about August 08,2014 and at times prior to, employees were exposed to a fall hazard of approximately 20 feet in height from a Humphrey Manlift Elevator, Serial #: S-3467, Model #: 14S GHC-3-PA while operating a manlift without posted instructions on the proper use; such as but not limited to signs stating: "Face the Belt", "Use the Handhold", "To Stop, Pull Rope in Direction of Travel".

29 CFR 1910.68(e)(1): All manlifts were not inspected by a competent designated person at intervals of not more than 30 days: At the Humphrey Manlift Elevator, On or about August 08, 2014 and at times prior to, employees were exposed to a struck-by hazard from descending steps while riding/operating the Humphrey Manlift Elevator, Serial #: S-3467, Model #: 14S GHC-3-PA, which had not been periodically inspected, and automatic safety stops were not functioning to shut off the electric power and apply the brake when a loaded step passes the bottom landing.
