Employees performing tool changes, servicing, and maintenance on machines such as, but not limited to coil spring winding machines A16 and HT-3, are not protected from unexpected start up or hazardous energy releases. Employees were performing setup work on spring winding machines that were energized and employees were not protected from unexpected startup. 29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:    The employer did not ensure that employees affixed lockout or tagout devices to energy isolating devices on machines where employees were performing tool changes, servicing, and maintenance activities.     a) An employee working on Machine A16, a Torrington Precision spring coiler (Serial 65474), did not affix a lockout or tagout device to the energy isolating device on the machine where the employee was performing setup. The machine was operational, and the employee was reaching into a side door where the employee's hand was exposed to in-running gears.     b) An employee working on Machine HT-3, a Herdon spring coiler, did not affix a lockout or tagout device to the energy isolating device on the machine where the employee was performing setup. The machine was operational, and the employee was exposed to the unexpected activation of the articulating arms.      Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c)

Abatement certification for this item is required in accordance with the requirements of 29 CFR 1903.19(c).
