29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) On July 2, 2014 and at times prior thereto; Southeast area of building; employees framing a new residential home were observed working on the roof area (outer wall) without a personal fall arrest system in place to prevent them from falling 40-45 feet to ground level

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On July 2, 2014 and at times prior thereto; Southeast side of residential home; the ladder used to access the work area (porch roof) was 36 inches below the working platform exposing them to a fall hazard of 12 feet to ground level while accessing or stepping off the ladder to the platform.      Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step:   On July 2, 2014 and at times prior thereto; Southeast side of residential home; the step ladder used to access the work area (porch roof) was 36 inches below the working platform having the employees to use the top step and exposing them to a fall hazard of 12 feet to ground level while accessing or stepping off the ladder to the platform.      Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
