29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: (a)GP Concrete Inc. at 5181 Andes St., Denver, CO 80249: On and before August 9th, 2014, the employer did not ensure that employees were protected from projectiles hazards to the eyes when an employee was operating a concrete saw. The condition exposed employees to the hazards of eye injuries.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard was introduced into their work area: (a)GP Concrete Inc. at 5181 Andes St., Denver, CO 80249: On and before August 9th, 2014, the employer did not ensure that employees were trained about the silica hazards in the workplace, and the need for respirators, when using cement saws. Condition exposed employees to respiratory and silica hazards.
