29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  a) Portable electric shop fan near North exit door in welding shop; On or about August 12, 2014 and times prior the employer exposed his employees to electric hazards in that a 120 V cord was improperly installed into a male plug. The outer insulation had been cut back so far, leaving the inner conductors in the area of the male plug that squeezes the cord and acts as strain relief, with the outer insulations cut back so far the outer diameter of the cord was too small for the plug to cinch to in order to provide strain relief.
