29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide:      On July 15, 2014 and at times prior thereto; side of the building; employees were using a four inch board as a walkway from one working platform to the other exposing them to a 30 foot fall to ground level.            Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end    On July 15, 2014 and at times prior thereto; side of building; employees were working off a pump jack scaffold which had the end of the working platform exceed 60 inches beyond the support creating a potential fall hazard of 30 feet to the ground level.         Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.451(b)(6): On scaffolds where scaffold planks were abutted to create a long platform, each abutted end did not rest on a separate support surface:    On July 15, 2014 and at times prior thereto; side of the building; employees were using a four inch board as a walkway from one working platform to the other. The abutted/overlapping ends were not set over a support member exposing the employees to a fall hazard of 30 to ground level.         Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
