The documentation did not specify each type of signaling (e.g. hand signals, radio signals, etc.) for which the signal person meets the requirements of paragraph (c) of this section. a/ Army Construction, LLC./105 Hamilton Ave., Staten Island, NY 10301: The employer did not complete and/or provide documentation indicating that the qualification requirements were met for employee providing hand signals while loading/unloading roofing materials onto a building roof that was approx. 4 stories high. Condition observed on/or about 7/30/14.
