29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: a. On or about August 5th 2014, an employee installing fence poles with a Powered Industrial Truck was exposed to severe electrical shock injuries, burns and or death while working within 10 feet of overhead power lines energized with 7,620 volts.
