29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative   a)  Handle Bag Machine #405; On or about August 12, 2014 and at times prior thereto the employer exposed employees to potential hazardous energy sources in that employees were allowed to clean the Handle Bag 405 Machine while only relying on control circuitry (E-Stop Button / Keyed Stop Button) as energy isolating devices which is prohibited by the standard.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):   a)  Handle Bag Machine #405; On or about August 12, 2014 and at times prior thereto the employer exposed employees to potential hazardous energy sources in that the written procedures for the locking out of the Handle Bag 405 Machine did not have clear and specific steps for isolating, securing, and verifying that the hazardous energy had been controlled.

29 CFR 1910.147(f)(1): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, equipment or component thereof, the sequence of actions were not followed in accordance with 29 CFR 1910.147(f)(1)(i)-(v):  a)  Module B - Handle Bag Machine 405; On or about August 12, 2014 and at times prior thereto the employer exposed employees to the unexpected energization of the B Module paste applicator in that while an employee was cleaning that area the machine was jogged to another position. The employer failed to develop or use procedures to safely position the machine in conjunction with service and maintenance activities in accordance with the standard.
