29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:  Well site:  On or about August 6, 2014 and times prior to, employees had replaced a service rig engine that had failed during an oil/gas well operation with the rig still erected on the well.  One of the employees was not using or provided with flame resistant clothing.  This exposed the employee to the hazards of explosion and/or fire.
