29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: On or about August 13, 2014 at a job site located at 4209 Jefferson Street, Medina, OH 44256, during construction activities including but not limited to installing vinyl siding from an elevated platform, the employer did not ensure employees working from an elevated platform were protected from fall hazards approximately 12 feet by a fall protection system/device. SIMMONS BROTHERS CORPORATION WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 29 CFR 1926.451(G)(4)(i), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314537242, CITATION NUMBER 01, ITEM NUMBER 001, AND WAS AFFIRMED AS A FINAL ORDER ON JUNE 29, 2010 WITH RESPECT TO A WORK PLACE LOCATED AT 18891 PEARL ROAD, STRONGSVILLE, OHIO 44136
