29 CFR 1910.22(b)(1): Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repairs, with no obstruction across or in aisles that could create a hazard. Warehouse - On or about August 13, 2014, in the warehouse, employees were required to operate powered industrial vehicles. The aisle ways in the southwest corner of the warehouse were not kept clear (blocked by overstock). Employees were thereby exposed to the hazards associated with blocked aisles and passageways.

29 CFR 1910.157(c)(4): The employer shall assure that portable fire extinguishers are maintained in a fully charged and operable condition and kept in their designated places at all times except during use. Warehouse - On or about August 13, 2014, the employer did not ensure that all portable fire extinguishers were kept in their designated places at all times. One (1) fire extinguishers in the west corridor exit of the warehouse was not present at the designated location.
