29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  At this excavation site, two employees bolted a brace to a sixteen inch water pipe and did not have a means of egress from a trench that was approximately seven feet in depth.  The exposed employees exited the trench by climbing on top of the pipe and being pulled out by other employees.

The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i):  At this excavation site, employees bolted a brace around a  sixteen inch water pipe while inside an unprotected excavation seven feet deep by nine feet wide by approximately nine feet in length.  The walls of the trench were near by vertical; the employees were exposed to hazards associated with a cave-in.
