29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: (a) On or about August 12, 2014, H.W. Houston Construction Company, as controlling and exposing employer, did not ensure that its employees and the employees of DS Masonry Inc., subcontractor, were protected from shock and other electrical hazards from the cord that connected the secondary construction trailer to the temporary electrical panel on the east side of the auditorium, in that the cord insulation was not connected to the fitting thus placing strain directly on the live conductors. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
