29 CFR 1910.184(e)(1): Alloy steel sling(s) did not have permanently affixed durable identification stating size, grade, rated capacity, and reach: a. In the Tanaka Laser Area an Alloy Steel Chain Sling that did not have identification attached was being utilized for lifting. Employees were exposed to a struck-by hazard. b. In the Fabrication Shop at Hoist #10 two Alloy Steel Chain Slings that did not have identification attached were being utilized for lifting. Employees were exposed to a struck-by hazard. c. In the Fabrication Shop at Hoist #8 an Alloy Steel Chain Slings that had a identification that was no longer legible attached was being utilized for lifting. Employees were exposed to a struck-by hazard.

29 CFR 1910.184(e)(3)(i): A thorough periodic inspection of alloy steel chain slings was not made on a regular basis and/or were done at intervals greater than once every 12 months: Throughout the plant - approximately forty-four alloy steel chain slings utilized for lifting steel production items had not been subjected to the required thorough inspection within the last twelve (12) months. Employees were exposed to the hazard of being struck-by the loads being lifted.

29 CFR 1910.184(e)(4): The employer did not ensure that before use, each newly repaired, or reconditioned alloy steel chain sling, including all welded components in the sling assembly, was proof tested by the sling manufacturer or equivalent entity, in accordance with paragraph 5.2 of the American Society of Testing and Materials Specification A391-65, which is incorporated by reference as specified in �1910.6 (ANSI G61.1-1968). The employer did not retain a certificate of the proof test and did not make it available for examination. The employer did not ensure that newly repaired and/or reconditioned alloy steel chain slings were proof tested and did not ensure that a certificate of that proof test was created and maintained. Employees were exposed to the hazard of being struck-by the loads being lifted.
