29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees:    a) Future site of Wallace Rd., Mount Holly, NJ:  On or about 8/12/2014 four C H Excavating employees were working in a trench excavation that was between 4.6 and 4.9 feet deep.  The employer did not provide a means of egress causing the employees to climb the excavation walls, creating a potential cave-in hazard.
