29 CFR 1910.332(a): Employees who face the risk of electric shock were not trained in and familiar with any electrically related safety practices required by 29 CFR 1910.331 through 29 CFR 1910.335 that pertain to their respective job assignments: On or about August 13, 2014, at the Balon Corporation the employer did not provide adequate training to an employee who was required to repair and replace electrical equipment. The employee was exposed to hazards caused by unexpected start-up of machines or equipment.

29 CFR 1910.333(b)(2): While an employee was exposed to contact with parts of fixed electric equipment or circuits which were deenergized, the circuits energizing the parts were not locked out or tagged or both in accordance with the requirements of this paragraph (b) of 29 CFR 1910.333(b)(2): On or about August 13, 2014, at the Balon Corporation the employer did not provide lock-out/tag-out equipment to an employee who was required to repair or replace electrical equipment. The employee was exposed to electric shock hazards from unexpected start-up of energy sources.
