29 CFR 1926.451(g)(1): 29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1m) above a lower level was not protected from falling to that lower level in accordance with paragraphs (g)(1)(i) through (vii) of this section:            On or about 08/08/2014, employees working from a ladder-jack scaffold at a height of 20(+) ft. during the tear off and re-roofing of a residential property located at 771 Youngstown-Kingsville Rd. in Vienna, OH, were not protected from a fall hazard by a personal fall arrest system or devices.
