29 CFR 1926.451(e)(1):  Scaffold platforms that were more than two feet (0.6m) above or below a point of access were not provided with a means of access:    (a)Diversified Masonry at  2785 and 2795 N. Speer Blvd., Denver, CO 80211:  On August 14, 2014 on the south side of building A and the southeast elevation of building B, the employer did not ensure a ladder was used to access the upper levels of the scaffold while it was being erected.  Without the ladder the employee climbed the bracing.  This condition exposed the employee to a fall hazard of approximately 12 feet on building A and 18 feet on building B.
