29 CFR 1910.136(a): The employer did not ensure that each affected employee uses protective footwear when working in areas where there is a danger of foot injuries due to falling or rolling objects, or objects piercing the sole, and where such employee's feet are exposed to electrical hazards:   Worcester Academy, Worcester, MA  On June 23, 2014, an employee was not wearing foot protection while lifting a 600lb commercial stove up a flight of stairs.  The stove slipped off a stair exposing the employees feet to crushing hazards.

29 CFR 1910.138(a): The employer did not select and require employees to use appropriate hand protection when employees hands were exposed to hazardous condition(s):  Worcester Academy, Worcester, MA  On June 23, 2014, an employee was not using hand protection while lifting a 600lb commercial stove up a flight of stairs.  The stove leg slipped off of a stair cutting off the fingertip of an employee still holding one side.
