29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):   On or about August 14, 2014, an employee was exposed to falls approximately 13 feet in height to the unpaved ground level below from a scaffold, which had only 3 out of 6 possible planks installed while employee was cleaning brick mortar joints.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:  On or about August 14, 2014, an employee was exposed to fall hazards of approximately 13 feet in height to unpaved ground below while cleaning brick mortar joints on an exterior wall from a scaffold that did not have mud sills installed under support poles.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces and side brackets were used as a means of access:     On or about August 14, 2014, employees were exposed to falls approximately 13 feet in height to the ground level below, when working from the work platform of the scaffold, which did not have a ladder for access and egress to and from the scaffold platforms, while cleaning brick mortar joints.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:   On or about August 14, 2014, an employee was exposed to fall hazards greater than 10 feet in height to unpaved ground level below when working from a scaffold that did not have guardrails installed, while cleaning brick mortar joints.
