29 CFR 1926.760(d)(2): Fall arrest system components were not used in fall restraint systems and did not conform to the criteria in 29 CFR 1926.502 (see Appendix G): On or about August 13, 2014, employee(s) were exposed to a fall of greater than 15 feet, as they were not wearing harnesses and lanyards, or provided any other form of fall protection, while spot welding members on the roof of a steel erection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee that enabled them to recognize the hazards of falling and did not train each employee in the procedures to be followed in order to minimize these hazards: Roof of steel erection building. On or about August 13, 2014, employees were exposed to a fall of greater than 15 feet to the ground level below, as they were not effectively trained in the hazards of fall protection, while they worked spot welding members on the roof of a steel erection.
