29 CFR 1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized:     a.  5500 United Drive SE, Smyrna GA - Safety-related work practices were not used and available to employees who worked on or near live electrical parts, in that employees were exposed to shock and arc flash/blast hazards because employees were not provided with personal protective equipment.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:    a. 5500 United Drive SE, Smyrna GA  - The employees were not provided with personal protective equipment commensurate with the parts of the body exposed to shock and arc flash/blast hazards thereby exposing the employees to shock arc flash/blast hazards.

29 CFR 1910.333(a)(1): "Deenergized parts." Live parts to which an employee may be exposed shall be deenergized before the employee works on or near them, unless the employer can demonstrate that deenergizing introduces additional or increased hazards or is infeasible due to equipment design or operational limitations. Live parts that operate at less than 50 volts to ground need not be deenergized if there will be no increased exposure to electrical burns or to explosion due to electric arcs     a.  5500 United Drive SE, Smyrna GA - Live parts to which an employee may be exposed were not deenergized or other authorized means used for employee protection from shock and arc flash/blast hazards before the employee worked on or near live parts.
