29 CFR 1917.112(b)(1): Guardrails were not provided at locations where employees were exposed to floor or wall openings or waterside edges, including bridges or gangway-like structures leading to pilings or vessel mooring or berthing installations, which presented a hazard of falling more than 4 feet or into the water. Pier 3, M/V Horizon Tacoma, crane cab, upper access landing: On or about 13 August 2013 and times prior there to, the railings on the crane cab access deck had two gaps in the railing measured at approximately 10 1/2 inches. This condition exposed employees to a fall hazard of approximately 60 feet.

(a) Pier 3, M/V Horizon Tacoma, Diesel house: On or about 13 August 2013 and times prior there to, Engine "A", serial number 2WB11151 and Engine "B", serial number 2WB11274, did not have fan pulley guards installed. This condition exposed employees to a caught in and struck by hazard. (b) Pier 3, M/V Horizon Tacoma, Machinery House: On or about 13 August 2013 and times prior there to, the Dayton air compressor, model number 3KV80A, was missing the pulley guard. This condition exposed employees to a caught in and struck by hazards.
