29 CFR 1910.134(c)(2)(i):  The employer did not provide the respirator users with the information contained in Appendix D of this section ("Information for Employees Using Respirators When Not Required Under the Standard"):  a)    Jobsite: Forever 21 Monroeville Mall, on or about August 12, 2014 -  Employees voluntarily used tight-fitting respirators, 3M 8210 Plus particulate filtering face pieces during flooring operations.    The employer did not provide employees with Appendix D of the respiratory standard.

29 CFR 1910.1200(e)(1): The employers did not maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met:  a)    Jobsite: Forever 21 Monroeville Mall, on or about August 12, 2014 -  Employees used various tile grouts and flooring adhesives such as but not limited to: KER 200 Keracolor S, which contained Portland cement and silica; Ultrabond ECO 980 which contained limestone and calcium oxide; and Ultraflor, a corrosive, which contained silica sand and Portland cement during new flooring installation.    A written hazard communication program was not maintained on the jobsite site to be referenced by employees.

29 CFR 1910.1200(g)(8): The employer did not maintain in the workplace copies of the required material safety data sheets for each hazardous chemical, and did not ensure that they are readily accessible during each work shift to employees when they are in their work area(s):   a)    Jobsite: Forever 21 Monroeville Mall, on or about August 12, 2014 -  The following safety data sheets were not maintained at the jobsite: KER 200 Keracolor S, which contained Portland cement and silica; Ultrabond ECO 980 which contained limestone and calcium oxide; and Ultraflor, a corrosive, which contained silica sand and Portland cement.    Employees used various tile grouts and flooring adhesives to install new floors in the Forever 21 store front.
