OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to flying debris and a crushing hazard due to over pressurization of storage vessels: a) C Cellar: Storage vessels, with improperly installed 15 PSI "boiler" pressure relief valves, were not operated, maintained and inspected as pressure vessels which resulted in an over pressurization and catastrophic failure of tank #62, on or about August 13, 2014. Abatement Certification Required.
