OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to combustible dust deflagration or other fire hazards while working at or near unsafe wood dust collection systems. (a) Mill Room, On or about August 20, 2014, employees were exposed to deflagration and fire hazards resulting from the ignition of combustible wood dust inside the dust collection ductwork, ignited from a static electric charge.

(b) Plant facility, Final Finish Area, appropriate hand protection was not provided when applying latex paint, creating a dermatological hazards. (c) Plant facility, Roof Decking Insulation Area, appropriate hand protection was not provided when installing insulation, creating a dermatological hazard.
