29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a)  At the jobsite, on the roof, on or about 8/14/14:  Employees were working on a 5:12 pitch roof, with an approximate ground to eave height of 25 feet without fall protection of any kind.  b)  At the jobsite, on a portico roof, on or about 8/14/14:  An employee was working on a low slope roof over a doorway, approximately 10 feet from ground without fall protection of any kind.  Youker Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(10), which was contained in OSHA inspection number 948249, citation number 1, item number 1a, and was affirmed as a final order on 1/17/14, with respect to a workplace located at 1333 Buckley Road, Syracuse, NY.  Abatement documentation must be submitted for this item.
