On or about August 13, 2014 and at times prior to, employees working on a commercial building were exposed to head injury while working in areas where overhead work was in progress such as but not limited to employees working on scaffolding above and roof work above.

On or about August 13, 2014 and at times prior to, an employee working on a scissor lift climbed out to install drywall studs and was not protected from falling by the use of fall protection.
